This does not include the cost of what the government calls &#8216;abnormal&#8217; health tourism, nor the costs of illegal migrants.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Health tourists are non-UK nationals who target the NHS because they will often get better treatment than at home \u2014 and for free, either by \u2018flying in and flying out\u2019 or through existing registration. By their very nature they are difficult to identify and quantify because they are likely to make efforts to conceal their true eligibility status or are not flagged up in the system.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em><strong>Britain is among the worst countries in Europe at extracting payments from foreign patients&nbsp;and&nbsp;four in five hospitals do not expect to start recouping more money <\/strong><\/em>(see House of Commons Public Accounts Committee&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/publications.parliament.uk\/pa\/cm201617\/cmselect\/cmpubacc\/771\/771.pdf\" target=\"_blank\">report<\/a>).<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Use of the NHS\u2014by foreign visitors who&#8217;ve ended up being treated while in England, for non-permanent residents and illegal migrants \u2014is estimated to cost just under <strong>\u00a31.74 billion a year<\/strong>. See Department of Health <a rel=\"noreferrer noopener\" href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/331623\/Impact_assessment.pdf#page=7\" target=\"_blank\">assessment<\/a>, which broke down the costs as follows: <ul><li><strong>\u00a3340m<\/strong> for EEA visitors and non-permanent residents to the UK (excluding expats). While the UK was a member of the EU, the government estimated that approximately \u00a3180m of this was recoverable through the European Health Insurance Card (EHIC), with the remainder potentially recoverable through S1 (\u00a360m) and other arrangements. It was estimated that the NHS recovered around \u00a350m in one year, less than 20% of the total potentially recoverable amount.<\/li><\/ul><ul><li><strong>\u00a31,070m<\/strong> for non-EEA visitors and temporary migrants to England. Of this approximately 14% (around \u00a3156m) was thought to be potentially, currently chargeable because the total gross expenditure includes both the costs of non-chargeable services A&amp;E and primary care and those individuals who are currently not chargeable due to being ordinarily resident or otherwise exempt.<\/li><\/ul><ul><li><strong>\u00a3330m <\/strong>for illegal migrants<ul><li><strong>Total = \u00a31.74 billion<\/strong><\/li><\/ul><\/li><\/ul><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>In contrast, the cost of those&nbsp;who had traveled to England purely to receive urgent treatment and regular visitors who were described as &#8220;taking advantage&#8221; of the system by registering for GP services and other NHS services to which they were not entitled was estimated at up to <strong>\u00a3300 million per year<\/strong><\/em>. See Department of Health <a rel=\"noreferrer noopener\" href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/331623\/Impact_assessment.pdf#page=7\" target=\"_blank\">assessment<\/a><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Figures for 2015\/16 show hospitals only managed to invoice patients for \u00a3289 million and much of this was never repaid (Public Accounts Committee&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/publications.parliament.uk\/pa\/cm201617\/cmselect\/cmpubacc\/771\/771.pdf\" target=\"_blank\">report<\/a>, 2017). Government studies also suggest that the NHS is also simply failing to charge when it is supposed to\u2014recouping only a fraction of what <a href=\"https:\/\/www.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/331623\/Impact_assessment.pdf#page=7\">should be around \u00a3340m from other countries<\/a>.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>GPs were also found to be doing too little to flag up those who should be charged. <em><strong>Britain is among the worst countries in Europe at extracting payments from foreign patients&nbsp;and&nbsp;four in five hospitals do not expect to start recouping more money.<\/strong><\/em><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Surgeon Dr J Meirion Thomas has&nbsp;<a rel=\"noreferrer noopener\" href=\"http:\/\/www.dailymail.co.uk\/debate\/article-5010685\/J-MEIRION-THOMAS-Health-tourism-gaping-wound-NHS.html\" target=\"_blank\">described<\/a>&nbsp;health tourism as a \u2018gaping wound\u2019 in the NHS. He says abuse is particularly bad in maternity and cancer wards.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>A relevant question is:&nbsp;<em><strong>Do those saying that this is not a problem believe that it should be the UK\u2019s responsibility to extend free health care to the whole world?<\/strong><\/em>&nbsp;<\/li><\/ul>\n\n\n\n<p><strong>Contextual points:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Unsurprisingly, \u2018health\u2019 is currently the public\u2019s top concern (YouGov issue tracking poll), with immigration behind the economy in third place (on par with the environment).<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>A UK-wide poll of just over 1,000 adults by&nbsp;<a rel=\"noreferrer noopener\" href=\"\" target=\"_blank\">Ipsos Mori<\/a>&nbsp;for the BBC showed that&nbsp;<strong>74%&nbsp;<\/strong>of respondents supported increasing charges for overseas visitors<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>From April 2017,&nbsp;NHS hospitals in England were made subject to&nbsp;a legal duty to charge overseas patients upfront for non-urgent care if they are not eligible for free treatment (see&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/www.bbc.co.uk\/news\/health-38876527\" target=\"_blank\">report<\/a>).<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>From December 2018, the immigration health surcharge was&nbsp;increased from \u00a3200 to \u00a3400 per year for non-EU nationals, with students and those on the&nbsp;Youth Mobility Scheme&nbsp;on the discounted rate of \u00a3300 per year (see&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/www.gov.uk\/government\/news\/increase-to-immigration-health-surcharge-gives-nhs-extra-funding\" target=\"_blank\">announcement<\/a>).<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>In mid-2019 the delegates at the British Medical Association voted overwhelmingly to stop charging overseas patients for their treatment at NHS hospitals.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Those illegally crossing the Channel in order to claim asylum, as well as 40,000 failed asylum seekers, are able to get immediate and full access to the NHS. Illegal immigrants are able to access GP services (see <a rel=\"noreferrer noopener\" href=\"https:\/\/questions-statements.parliament.uk\/written-questions\/detail\/2021-09-21\/%35%32%35%36%33\" target=\"_blank\">written Parliamentary Answer<\/a>, September 2021) despite the huge pressure and crisis which GP practices are facing due to skyrocketing demand. This is surely unfair to the millions of people who pay into the system and expect to get good NHS care when they need it. <\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Between 2010 and 2020, there were just under seven million new GP registrations by migrants in England, Northern Ireland and Wales (<a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/populationandmigration\/migrationwithintheuk\/datasets\/localareamigrationindicatorsunitedkingdom\" target=\"_blank\" rel=\"noreferrer noopener\">ONS local area migration indicators<\/a>). <strong><em>That is more than one every minute. <\/em><\/strong><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>The total additional cost of net migration for the NHS in 2014 was \u00a3360 million (\u00a3160 million for EU immigration and \u00a3200 million for non-EU immigration) &#8211; (See <a href=\"https:\/\/fullfact.org\/europe\/eu-immigration-and-pressure-nhs\/\" target=\"_blank\" rel=\"noreferrer noopener\">source<\/a>: NHS Five Year Forward View, Nuffield Trust calculations, May 2016). <\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Total NHS expenditure for the migrant population in 2014\/15 was \u00a318.6 billion (see Table 2 of our <a rel=\"noreferrer noopener\" href=\"https:\/\/www.migrationwatchuk.org\/briefing-paper\/381\" target=\"_blank\">fiscal analysis<\/a>).<br><\/li><li>Research suggests that immigration has been a huge overall annual fiscal cost (between <strong>\u00a34.3 billion and \u00a313 billion per year <\/strong>for 2014\/15 and 2016\/17 depending upon assumptions &#8211; see e.g. our <a rel=\"noreferrer noopener\" href=\"https:\/\/www.migrationwatchuk.org\/briefing-paper\/381\" target=\"_blank\">fiscal analysis<\/a>).<\/li><\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Health tourism drains the NHS of at least \u00a3300-400 million each year because hospitals fail to identify overseas patients or never send them bills.\u00a0 The government has estimated that \u00a3388 million is spent each year on EEA and non-EEA patients who should be paying for their care but are not identified or charged.
